WOMBAT Workshop: Women's & Babies Health and Wellbeing Improving Outcomes

Perth July 7

At our recent regional workshop in Perth we held a session on enhancing the research culture to support trial activity.

This session was enthusiastically received by participants who identified indicators of research culture, strategies to achieve these and measures of success or failure. The group answered the questions: "What is research culture and how do we know if we've got one?"

The five key indicators of research culture identified by the group were:

  • having a questioning attitude – not just accepting things done according to custom and practice and understanding that there is still much we do not know

  • support for staff to undertake research

  • systems which promote not hinder research

  • professional development opportunities for staff

  • a process for communicating the outcomes of research done in the hospital so that they influence care in that hospital

Innovative strategies to achieve this included changing the way that we talk about research so that it is more accessible to general staff, having time for research written into staff employment contracts, bringing research activities and the results of research into the mainstream communication channels of the hospital (such as the hospital newsletter) rather than in specialised research-focused publications, and for hospital research ethics committees to have a system of referral to mentors to resolve the problems highlighted in the ethics appraisal so that the research can be approved and undertaken.

A full list of the indicators and the strategies matrix is available from the WOMBAT website.

Trial Updates

Recruitment is Complete on the EECV2 Trial

The Early External Cephalic Version 2 (EECV2) Trial has completed recruitment. This study seeks to answer the primary question: For women with a foetus in breech presentation, does early external cephalic version (ECV) (at 34-35 weeks) versus delayed ECV (not before 37 weeks) increase or decrease the likelihood of CS? In addition, it will look at the secondary question: Is the risk of preterm birth (<370/7 weeks) lower or higher with early versus delayed ECV?

1543 women were recruited into this trial from 68 centres in 21 countries, including Australia – the number one recruiter. Thank you to all of our collaborators for ensuring the success of the trial! We anticipate results will be available in 2009.

PPROMT (Preterm Pre-Labour Rupture of Membranes Close to Term Trial) Update

The last 2 months have been the best yet for recruitment for the PPROMT trial. June saw the highest number of recruits ever with 28 new participants being recruited. Also, congratulations to Dr.José Luis Castaldi and Dr Marta Bertin from Hospital Penna, Argentina and Dr. Anna Dera at University of Medical Sciences, Poznan, Poland who have each recruited five participants in the past 2 months– well done!

PPROMT also held an International Collaborators Meeting on the 26th and 27th of June at Sydney University. This highly successful workshop brought together researchers from Argentina, Brazil, Romania, Egypt, Norway, United Kingdom, New Zealand and Australia to discuss the trial. Much of the meeting was devoted to considering the language used when discussing trials with women and their families. All in attendance enjoyed the meeting and left with great enthusiasm to continue their recruitment into the study.

New Reviews in the Cochrane Library and the Cochrane Impact Factor

Information on the 2007 Impact Factor for the Cochrane Library is now available. The impact factor is a tool for ranking, evaluating and comparing journals. It is a measure of the frequency with which an ‘average article’ in a journal has been cited in a particular year.

Issue 3, 2008 of the Cochrane Library is now available and features the following new titles:

Neonatal Group
Protocols

Dilute versus full strength preterm formula in preterm or very low birth weight infants
Heliox for prevention of morbidity and mortality in ventilated newborn infants
Propofol for procedural sedation/anesthesia in neonates

Reviews
Chest physiotherapy for reducing respiratory morbidity in infants requiring ventilatory support

Pregnancy and Childbirth Group
Protocols
Cervical assessment by ultrasound for preventing preterm delivery
Early versus late initiation of epidural analgesia for labour
High dose versus low dose oxytocin for augmentation of labour
High feedback versus low feedback of routine prenatal ultrasound for improving outcomes
Interventions for preventing mastitis after childbirth
Medical treatments for incomplete miscarriage (less than 24 weeks)
Pacifier use versus no pacifier use in breastfeeding term infants for increasing duration of breastfeeding
Screening for gestational diabetes mellitus for improving maternal and infant health
Transcutaneous electrical nerve stimulation for pain relief in labour

Reviews
Amniotic fluid index versus single deepest vertical pocket as a screening test for preventing adverse pregnancy outcome
Antenatal breast examination for promoting breastfeeding
Interventions for ketosis during labour
Methods of delivering the placenta at caesarean section
Rapid versus stepwise negative pressure application for vacuum extraction assisted vaginal delivery
